I think the S is pointless, people are going to say Elizzzzabeth either way. But I love Elizabeth, its my middle name and will probably be my daughter's middle also if I ever have one.
I usually don't like unusual spellings but I really prefer Elisabeth over the "z" spelling!! I would try to aim for more creative middle names if I were using a common name like Elisabeth, though.
I really like Elisabeth Ann. I know a few Elisabeth's, actually. Well, maybe only one. But I also then only know one Elizabeth. Hmmm...but it's not a crazy different spelling. It's not like Ilyzabith or anything.
Elisabeth is a vey traditional name and, in my opinion, it just sounds so dull and boring with another very traditional name like Ann. So I would go with Elisabeth Renee.
